New Student-Athletes
During the Pre-participation Examination the WOU Athletic Medical Director will evaluate and classify all known pre-existing Injury/Conditions of the student-athlete. To avoid any delays in being cleared to participate please bring all medical documentation including your treating physicians’ recommendations for all current or recent injuries. Western Oregon University is not responsible for any pre-existing injuries or medical conditions of the student-athlete prior to the pre-participation examination. Any medical costs for such injuries or conditions will be the responsibility of the student-athlete and/or parent. This includes the costs associated with any diagnostic test or medical procedures that the Athletic Medical Director may require before full participation is granted.

The following forms are in pdf format. They must be completed on a computer. You cannot save data typed into the forms. Please print your completed forms and submit them to the WOU athletic training staff on the day of your pre-participation examination. Please print or make copies for your personal file. No hand written forms will be accepted.

The following 4 steps must be completed prior to your scheduled Pre-participation Examination.  Please bring all completed forms with you on your scheduled pre-participation examination date.  Failure to submit all of the required forms in the format request will result in you not recieving medical clearance to participate on the date of your scheduled pre-participation examination:

Step 1
The following forms require the signature of your primary insurance policy owner. Your primary insurance policy owner in most cases is your parent or legal guardian. Please note: when completing the Intercollegiate Athletic Accident Insurance Verification Questionnaire a copy of your primary insurance card is required. Complete, Print, Sign and Submit the following forms:
                WOU Student-Athlete Health Insurance Policy
                • Insurance Verification Questionnaire
                • WOU Medical Expense Policy


Step 2
Complete, Print, Sign and Submit the following forms:
               • New Student-Athlete Medical History
               • Cardiovascular Evaluation
               • WOU Sports Medicine Policy

Step 3

Step 4
ImPACT Test
With the exception of Cross-Country and Track student-athlete, all student-athletes and cheer participants are required to complete an ImPACT Baseline Test before they will be allowed to participate in any WOU associated voluntary activities and/or NCAA Countable Athletically Related Activities. The ImPACT Test is part of the WOU approach to management of sport concussion. Please click on the following hyperlink for specific instructions on how to complete the test:

Men's and women's track and field teams win 2008 GNAC outdoor championships.

Baseball wins 10th consecutive conference championship.

Volleyball wins first GNAC championship (2008).

Men's track and field team wins third consecutive GNAC indoor championship (2010).
