Late Returning Student-Athletes
This classification of student-athletes consists of individuals who received medical clearance to participate in WOU “NCAA Reportable Accountable Athletic Activity” the previous academic year, and were not present on the institutional declared date of the PPE.
 
All ‘Returning Student-Athletes’ must receive medical clearance annually before every competitive sports season from the Western Oregon University Sports Medicine Staff.  If there has been a change in your health history or if you received an evaluation or treatment from any physician who is not a member of the Western Oregon Sports Medicine Staff please bring all medical documentation including your treating physician’s recommendations for all current or recent injuries or illness. 

The following forms are in pdf format. They must be completed on a computer. You cannot save data typed into the forms. Please print your completed forms and submit them to the WOU athletic training staff on the day of your pre-participation examination. Please print or make copies for your personal file. No hand written forms will be accepted. 
 
Medical Clearance steps
The following 8 Medical Clearance steps must be completed in the correct stepwise order:
 
Medical Clearance Step 1
The athletic training staff and WOU NCAA Compliance and Eligibility Officer must first receive notification from the coaching staff of your addition to the team roster.
 
Medical Clearance Step 2
Schedule an appointment with the WOU NCAA Compliance and Eligibility Officer to complete all eligibility and pre-participation paperwork. Your coach will provide you with the information necessary to help you complete this task.
 
Medical Clearance Step 3
Meet with the WOU NCAA Compliance and Eligibility Officer and complete your eligibility and pre-participation paperwork. Once you have been declared by the WOU NCAA Compliance and Eligibility Officer that you are eligible to participate in an “NCAA Reportable Accountable Athletic Activity” your name will be posted on the team roster as being eligible to participate.

Complete the 3 Pre-participation steps as outlined. 

Pre-participation steps
The following 3 steps must be completed prior to your scheduled Pre-participation Examination.  Please bring all completed forms with you on your scheduled pre-participation examination date.  Failure to submit all of the required forms in the format request will result in you not receiving medical clearance to participate on the date of your scheduled pre-participation examination:

Step 1
The following forms require the signature of your primary insurance policy owner. Your primary insurance policy owner in most cases is your parent or legal guardian. Please note: when completing the Intercollegiate Athletic Accident Insurance Verification Questionnaire a copy of your primary insurance card is required. Complete, Print, Sign and Submit the following forms:
           • WOU Student-Athlete Health Insurance Policy
           • Insurance Verification Questionnaire
           • WOU Medical Expense Policy

Step 2
Complete, Print, Sign and Submit the following forms:
             • Returning Student-Athlete Medical History
             • WOU Sports Medicine Policy

Step 3

Medical Clearance Step 5
Schedule an appointment with the WOU Athletic Training Staff to obtain a pre-participation examination.  Football  and Track student-athletes will schedule an appointment with Matt Bowman in the OPE Athletic Training Room (503-838-8343); all other student-athletes will schedule an appointment with Jennifer O’Sullivan in the NPE Athletic Training Room (503-838-8251). Appointments will not be scheduled until you have completed the 3 Pre-participation steps and the athletic training staff has verified with the NCAA Compliance and Eligibility Officer that you are eligible to participate in an “NCAA Reportable Accountable Athletic Activity”.
 
Medical Clearance Step 6
Bring all completed pre-participation forms with you on your scheduled pre-participation examination date.  Failure to submit all of the required forms in the format requested will result in you not receiving medical clearance to participate on the date of your scheduled pre-participation examination and a new appointment will need to be scheduled. All required documentation and medical evaluations will then be used by the Athletic Medical Director to determine your eligibility for participation.

Medical Clearance Step 7
After you have received medical clearance to participate you must schedule a time with the WOU athletic training staff to complete a NCAA mandated educational session. You will not be eligible to participate in an “NCAA Reportable Accountable Athletic Activity” until you have completed the NCAA educational session.
 
Medical Clearance Step 8
Attend your scheduled NCAA mandated educational session. At the completion of the session you will be declared eligible to participate in all WOU NCAA Reportable Accountable Athletic Activities. Notification of your status will be given to your coach.
