Try-Out Procedure for individuals who are NOT currently enrolled and attending class at WOU:
NCAA Requirements
- Prior to participation in a Western Oregon University (WOU) “try-out activity” all participants are required by the NCAA to undergo a medical examination or evaluation administered or supervised by a physician. The examination or evaluation must be administered within six months* prior to participation in the “WOU Try-Out Activity”. The medical examination or evaluation must include a sickle cell solubility test (SST), unless documented results of a prior test are provided to WOU or the participant declines the test and signs a written waiver of release.
* NCAA official interpretation of “within six months”: "within six months prior to a prospective
student-athlete's participation in practice, competition or out-of-season conditioning activities during
his or her senior year in high school or just completed participation at a college or university" . . . .
You can satisfy this requirement if you just finished your Senior year of high school or just
finished a season of participation at another college or university and you can provide a copy
of a medical examination or evaluation that was completed within six months to the start of your
"Traditional Sports Season".
- The NCAA requires that the medical examination or evaluation be administered or supervised by a physician (e.g., family physician). A nurse practioner whose state medical licensure allows for health care practice independent of physician supervision may complete the medical examination without supervision by a physician.
Please follow the steps below in a timely manner to facilitate your eligibility to tryout:

Download, complete and sign the “WOU Assumption of Risk Form”.
If you are under the age of 18, the WOU Assumption of Risk Form must be signed by your parent or legal guardian.

Provide proof of a medical examination or evaluation that meets the NCAA requirement listed above.
- The medical examination must include an evaluation of your personal and family history of cardiovascular disease along with an evaluation of your heart and lung sounds. Western Oregon University Athletics has accepted the standards established by the American College of Cardiology and the American Heart Association for pre-participation screening for sudden cardiac arrest. Both the American Heart Association and the American College of Cardiology recommend taking an athlete's family history using the AHA's 14-point questionnaire and conducting a physical examination to determine the athlete's risk of cardiovascular disease before the student-athlete participates in sports. PLEASE NOTE: If your try-out is successful and you are selected as a new student-athlete to join your respective sport team you will be required to submit a cardiac screen that has been completed on the "WOU AHA 14-point Examination Form"

Try-Out Step 3

Submit Completed Forms
Submit your completed forms including a copy of your current medical examination or evaluation to the Western Oregon University Athletic Coach who will be conducting the "try-out activity". All of your forms and medical examination will be shredded and destroyed following your try-out. If you want to recover your information prior to disposal please inform the WOU athletic coach or the WOU athletic training staff.
Submission of your forms DOES NOT qualify you to participate in a "try-out". A member of the WOU athletic training staff will review your medical examination and completed forms to determine if they satisfy the NCAA requirement. The Western Oregon University Athletic Coach who will be conducting the "try-out activity" will be notified by a member of the WOU athletic training staff of your eligibility to participate.
