Initial Year of Participation and NOT Present on Declared date of PPE
This category is defined as individuals who desire medical clearance to participate and were not present on the Institutional Declared Date of Pre-Participation Evaluation for their sport or desire medical clearance to participate in the voluntary off-season physical conditioning activity (summer strength and conditioning) that is monitored by a Western Oregon University athletics personnel prior to their sports pre-participation evaluation date. Student-athletes in this category are personally responsible for all financial cost associated with obtaining medical clearance.

NCAA and WOU Requirements
  1. All student-athletes who will begin their initial year of participation at Western Oregon University must receive medical clearance prior to participation that includes cardiac screening for sudden cardiac death.
  1. The cardiac screening must be administered by a medical doctor (MD), doctor of osteopathic medicine (DO) or a nurse practioner whose state medical licensure allows for health care practice independent of physician supervision may complete the medical evaluation without supervision by a physician.
  1. The medical examination must be administered within six months prior to participation.
  1. Western Oregon University Athletics has adopted the American Heart Association (AHA) 14-point comprehensive personal and family history questionnaire and physical examination as part of the pre-participation cardiac screening.
  1. The American Heart Association 14-point examination must be documented on the Western Oregon University AHA 14-point Examination form. The form can be downloaded by clicking on the following hyperlink: "WOU AHA 14-point Examination Form"


Please follow the steps below in a timely manner to facilitate your eligibility to participate:

Step 1
Obtain a cardiac screen from a medical professional that meets the NCAA and WOU requirements as outlined above. To ensure that the American Heart Association recommendations are followed the cardiac screen must be documented by your medical professional on the Western Oregon University AHA 14-point Examination Form. Step 2
Complete all the required medical forms on the Athletic Training Software, click on the following hyperlink for specific instructions: Initial Year of Participation and NOT Present on Declared date of PPE

Step 3
Upload your completed "WOU AHA 14-point Examination Form" to your Athletic Training Software account. Directions on how to upload your WOU AHA 14-point Examination Form are explained under the "eFiles" of the Athletic Training Software. **** Caution: Make sure you upload all 3 pages of the WOU AHA 14-point Examination Form

Step 4
Send an email to the staff athletic trainer once you have completed step 1 through step 3. Include in your email a contact phone number. The staff athletic trainers email is: wouatc@wou.edu Completion of all steps DOES NOT qualify you to participate. You will get a reply email from a staff athletic trainer with instructions on how to proceed.
