Initial Year of Participation and Present on Declared date of PPE.
This category is defined as individuals who will be present on the institutional declared date of the pre-participation evaluation for their respective sport. Currently the specific "Sport Report Dates" have not been established. When available the Institutional Declared Date of Pre-Participation Evaluations will be listed on this website. Please contact the Head Coach of your sport for more information.
 

Prior to your "Sport Report Date" and the Declared Date of Pre-Participation Evaluations all of your medical forms must be completed on the Athletic Training Software. Failure to submit your forms prior to the submission date could result in a delay in your ability to participate with your teammates. Complete the forms now and then report on your "Sport Report Date" for the Pre-Participation Evaluation.
