All student-athletes who will begin their initial year of participation at Western Oregon University must receive medical clearance prior to participation that includes a cardiac screen.

The WOU Sports Medicine Staff highly recommends student-athletes to complete the Cardiac Screen
(WOU AHA 14-Point Examination Form)
at home with their primary care provider or at urgent care then upload on your ATS Portal. This will help to expedite medical clearance during your report date. If one cannot be obtained at home, we will still provide this service during your scheduled PPE date.

NCAA MEDICAL EXCEPTION FORM
Some prescription medications are included on the NCAA Banned Substance List. Student Athletes who are taking these medications are required to provide medical documentation to receive NCAA Medical Exception Pre-Approval. See the following links for further information.

SUMMER CONDITIONING OR NOT PRESENT ON DECLARED DATE OF PPE?
If you will not be present on the declared PPE date, it is your responsibility to complete a physical by a Medical Doctor (MD), doctor of Osteopathic Medicine (DO), or a Nurse Practitioner (NP) whose state medical licensure allows for health care practice independent of physician supervision. This exam must be administered within six months prior to participation.
