Individuals who were medically cleared to participate at WOU during the previous academic year and will continue participation at WOU for another season will need to complete the forms requested of them on the checklist. See links below to assist with completing the correct forms.

*** IMPORTANT: All student-athletes who will be participating in their 3rd and 5th year of eligibility at WOU will repeat the American Heart Association's 14-point examination form and physical examination to determine the student-athletes risk of cardiovascular disease and cardiac sudden death
 

We are strongly recommending student athletes (3rd and 5th years) to complete the Cardiac Screen (AHA 14-Point Exam Form) at home with your primary care provider or at an urgent care and then upload it to ATS Portal. This will help to expedite medical clearance during your report date. If one cannot be obtained at home, we will still provide this service during your scheduled PPE date.
