STEP ONE:

Download, complete and print the "Pre-Participation Coversheet"

STEP TWO:
Download, complete, sign and print the "WOU Assumption of Risk Form". (If you are under the age of 18, the WOU Assumption of Risk Form must be signed by your parent or legal guardian.)

STEP THREE:
Provide proof of a Medical Examination or evaluation administered within SIX Months prior to the start date of the "Try-Out Activity" that satisfies the NCAA requirement and includes the following:
 

-- Evaluation of personal and family history of cardiovascular disease.
-- Evaluation of heart and lungs.
 
*** WOU has accepted the standards established by the American College of Cardiology and the American Heart Association for pre-participation screening for sudden cardiac arrest. Both the AHA and the ACC recommend taking an athletes family history using the AHA's 14-point questionnaire and conducting a physical exam to determine the athlete risk of cardiovascular disease before the student-athlete participates in sports. NOTE: if your try-out is successful and you are selected as a new student-athlete to join your respective sport team you will be required to submit a cardiac screen that has been completed on the "WOU 14-point Examination Form".

STEP FOUR:
Provide proof a sickle cell solubility test (SST).

   STEP FIVE:
Staple all forms required above together including a copy of your medical examination. 

Give completed packet to the WOU Sport Coach who will be conducting the Try-Out Activity.
 
Submission of your forms DOES NOT qualify you to participate in a "try-out". A member of the WOU Sports Medicine Staff will review your medical examination and completed forms to determine if they satisfy the NCAA requirement. The WOU Athletic Coach who will be conducting the "try-out" will be notified by a member of the WOU Sports Medicine Staff of your eligibility to participate.
