Required Forms and Procedure to obtain Medical Clearance to Participate:

The following forms are in pdf format. They must be completed on a computer. You cannot save data typed into the forms. Please print or make copies for your personal file. Please print your completed forms and submit them to the WOU athletic training staff on the day of your pre-participation examination. No hand written forms will be accepted.
